Chronic Disease Integrated Partnership Grants

The Chronic Disease Integrated Partnership Grants program is a Wellbeing SA initiative to support government, non-government organisations, General Practice, Primary Health Networks, Local Health Networks and the community working collaboratively to improve health outcomes and experiences for people living with chronic conditions.

Round 2 of the grants are now open, non-government non-profit organisations are invited to submit a grant application.

Projects must demonstrate how they will contribute to improved integration and quality healthcare for people living with chronic conditions through a partnership based approach.

Grant funded projects will ideally build on existing services, programs and initiatives to further develop or bring to scale. Proposals seeking to test new and innovative strategies, including use of new or emerging technologies, products and processes that support better healthcare integration for people living with a chronic condition will also be considered.

Applying organisations and/or project partners will need to describe how they will co-contribute to the project. This may be in the form of a financial or other in-kind contribution.

Grant recipients will not receive further or ongoing funding from Wellbeing SA. Applying organisations need to clearly articulate how project proposals will become sustainable or provide opportunity to inform or support future service design, system changes and improvements.

There is a total funding pool of $250,000 (GST exclusive) available and individual grants will be allocated up to $60,000 each (GST exclusive).
